Rochester Primary Care Network is a lead agency in the Rochester region to advance the availability of health and health-related services for the poor and underserved. RPCN will focus the community's attention and resources to address primary health care issues and will initiate partnerships with health care providers and others who share our commitments by:

• Improving quality of health care outcomes
• Expanding the availability of necessary services
• Improving the processes of care that support these necessary services
• Advocating for services that are socially, culturally and economically integrated into a single system of care
