Rushville Community Health Center, located in Yates County, New York, was founded in 1970, making it one of the most established Community Health Centers in the United States. Utica Community Health Center, located in the heart of the City of Utica, New York, is RPCN’s newest health Center. Utica CHC provides both Medical and Dental services to people of all ages. Livingston Community Health Center, located in Mt. Morris, New York, is a full service, state-of-the-art dental center, offering dental services to people of all ages. The site is currently accepting new Dental patients. All forms of health insurance are welcome, including Medicaid and Child Health Plus. Livingston CHC also accepts patients without insurance or with limited insurance. Sliding Fee discounts are available to those who qualify. Patients seeking medical primary care services will be connected with a local provider under contract with RPCN.

Lyons Community Health Center, located in Wayne County, New York, offers preventative dental services to people of all ages. The site is currently accepting new Dental patients. All forms of health insurance are welcome, including Medicaid and Child Health Plus. Lyons CHC also accepts patients without insurance or with limited insurance. Sliding Fee discounts are available to those who qualify. Patients seeking medical primary care services will be seen at Rushville Community Health Center.

Additionally, RPCN serves patients from Monroe County and the surrounding area through partnerships with community organizations, Eastman Dental Center, Rochester General Health System and Unity Health System. RPCN affiliates operate 19 urban health centers, including 5 school-based health centers, offering medical, dental, outreach and behavioral health services in neighborhoods across the city of Rochester.
