Mosaic Health Ilion was opened in the spring of 2016 to improve access to health care across the Mohawk Valley Region of New York. The center will help fill the gaps in primary and prenatal care and provide much needed access to chronic disease management and prevention services—especially for the underinsured. Access to primary care services should also help reduce the number of local hospital admissions. And because we know that getting to and from your appointment can be a problem, staff can help coordinate transportation to make sure you can keep your appointments.

In 2018, with financial support from the Community Foundation of Oneida and Herkimer Counties, the center was able to expand from three exam rooms to eight, build a larger waiting room, increase social work services, and add behavioral health and telehealth services. Now a full-service center, staff will be able to help additional patients in a more efficient manner. Extra staff have also been hired to accommodate patient growth. 

Patients are managed by a care team, which includes the providers you see at the center—such as your physician, physician assistant, nurse practitioners, and social worker care managers. Diagnostic laboratory, diagnostic radiology and pharmaceutical services will be provided directly and via formal contracts with local providers and pharmacies.

The care team will make sure you receive accurate and timely treatment and can be a source of guidance, support, education, and empowerment. They coordinate appointments and services and build strong relationships with other health care team members by keeping communication lines open with each other and the patient. They will also help you:

  • Offer support, guidance, and instruction

  • Learn about the health care system

  • Understand your diagnosis and treatment

  • Find available educational resources and supportive services

  • Get answers to insurance and financial concerns by connecting you to appropriate resources

  • Coordinate appointments and transportation assistance

  • Provide the answers you need and help you understand the information delivered

  • Make referrals to appropriate services

Dental Services are available to patients through internal self-referral to Mosaic Health Utica.  Access to dental services will be facilitated by on-site staff.  

The centers are currently accepting new patients. All forms of health insurance are welcome, including Medicaid and Medicare. The center also accepts patients without insurance or with limited insurance. Sliding Fee discounts are available to those who qualify.

Interpreter services are available.
