Notice to Affected Individuals

Mosaic Health is committed to having an effective Compliance Program, meaning we are dedicated to acting morally and ethically in everything we do as an organization. This includes compliance with our Mission, Vision and Code of Conduct as well as local, state and federal laws and regulations.

Our Code of Conduct outlines how we support this through our daily actions as staff, Board Members, contractors and volunteers. We have supplied a copy under Compliance Plans and Policies.

Mosaic Health is dedicated to creating an environment where open, honest communication is the expectation and not the exception. The Compliance Program encourages and promotes an environment where suspected fraud, waste, abuse and unethical or illegal activities can be anonymously reported by anyone without fear of retaliation. We want you to feel comfortable coming forward in instances where you believe violations of policies or misconduct have occured. The Compliance Program is committed to investigating every claim and recommending a warranted action plan as necessary.

Report a Compliance Concern

To file a complaint or compliance issue with Mosaic Health’s Compliance Officer:

  • Send a letter addressed to the Compliance Officer to our Corporate Headquarters: 1 S. Washington St. Rochester, NY 14614

All communication received will be kept confidential. Individuals reporting in good faith are protected under Mosaic Health’s non-intimidation and non-retaliation policies.
